Two piece,aluminum and plastic; 6-1/2 inches long; 4-1/4 inches high; plastic back with metal mounting plate and 12 instruction cards; four horizontal slot in aluminum holder accommodates one patient identification card and three nursing instruction cards; includes integral sotrage slot at back of holder for remainder of instruction cards; attaches to a variety of surfaces by use of screws,adhesive,or by two permanent magnets on back of holder; note: replacement reminder cards may be purchased ocally as required
